**Providence is calling a Manager - Population Health Support Team to Providence Health & Services in Torrance, CA.**

We are seeking a Manager - Population Health Support Team to oversee the daily duties and responsibilities of the Population Health Resource Specialists. The position oversees the supervisor(s) and staff on the team providing daily leadership and support for issues, concerns, and design of specific staff workflows. The position will also be a point person in responding to issues from providers and patients in all regions receiving support from the team. This position is a liaison between System providers, specialist offices, patients, multiple scheduling and modality (e.g., Laboratory) departments, and freestanding laboratory centers. This position also serves as an expert resource to Regional leaders, Physician Services providers, staff and patients in regard to the population health programs and related processes. Performs all duties in a manner that promotes the team concept and reflects the organizations mission and philosophy.

**In this position you will have the following responsibilities:**

+ Manages the daily operations and functions of the Population Health Support Service. Assesses and collaborates with the supervisor(s) in redistribution of duties to ensure timely and appropriate work tasks completion.

+ Manages the outreach program implementations by showcasing the team's processes and deliverables to internal customers. Acts as a resource to other System Physician Services, Regional Leadership, and clinical practices for developing and implementing Population Health Support programs.

+ Prioritize and coordinates activities of the Population Health Support Team to optimize the services in accordance with organizational goals and individual patient needs

+ Manages team's budget for overtime and team expenses

+ Establishes and maintains Clinical Services Outreach Team standards, policies and procedures. Serves as a resource to answer questions regarding correct procedures.

+ Communicates and collaborates with department leadership team and patient care team regarding daily operational issues.

+ Participates in problem resolution of routine or non-routine outreach requests or projects. Triages issues as appropriately to department leadership or appropriate caregiver in the larger patient care team.


**Required qualifications for this position include:**

+ Bachelor's Degree, or equivalent combination of education and experience.

+ **Nationally Certified** Medical Assistant (current).

+ 6 years of experience in a closely related field.

+ 4 years of experience in supervisory or leadership position.

+ Experience with electronic medical records and/or computerized billing systems.

+ Ability to manage day to day activities of staff to achieve successful outcomes and positive team dynamics

+ Ability to establish priorities, organize work, be detail oriented, and follow through.

+ Ability to identify and troubleshoot complex problems.

+ Demonstrated ability to properly plan and execute on project deliverables.

+ Demonstrated appreciation of team dynamics, motivation and coaching for successful team development.

+ Knowledge of medical terminology, disease processes, medical decision making, and prior experience in utilizing disease registries or similar databases.

+ Excellent verbal communication, negotiation, motivational interviewing / coaching, and interpersonal skills.

+ Demonstrated ability to effectively establish and maintain working relationships with peers and constituents at all levels of the organization.

+ Knowledge of medical terminology, and general knowledge of health care environment and delivery systems.

+ Computer skills including Microsoft Windows and Office Applications (Word, Access, and Excel).

+ Excellent typing skills (60 words/minute), computer keyboard, and mouse skills for expedited data processing.

+ Proficiency in data processing skills such as word processing and email. Knowledge and experience in utilizing databases and spreadsheets.

+ Ability to adapt work pace and thought process during urgent situations and under pressure.

+ Ability to perform multiple tasks simultaneously and work effectively with interruptions.

+ Demonstrated training and coaching skills, ability to provide technical descriptions to co-workers and external contacts.

+ Ability to be patient, caring and understanding in dealing with patients, or other external contacts, who may be confused, upset or difficult.

+ Appreciation of cultural diversity and sensitivity when interacting with diverse population groups.

**Preferred qualifications for this position include:**

+ Healthcare related degree.

+ 3 years of experience of direct care in primary care.

+ EPIC, CareManager, software experience.
